1. Assumption of Risk: I understand that Taekwondo is a physical sport and carries a risk of injury. I agree that MWS Taekwondo Academy and its trainers are not liable for any injuries sustained during training.

2. Performance-Based Training: I acknowledge that the academy is performance-oriented. Selection for any state, national, or international team is not guaranteed and depends solely on the student's performance and the judgment of the coaches.

3. Training Consent: I consent to rigorous, disciplined training methods deemed necessary by the coach to enhance performance, provided they adhere to safety regulations. I agree not to object to such training methods.

4. Code of Conduct: MWS Taekwondo Academy has a zero-tolerance policy for any form of sexual harassment or misconduct. Any such behavior will result in immediate expulsion.

5. Declaration: I declare that all the information provided in this form is true and correct to the best of my knowledge.
